Nicatto Health Dynamics (“Nicatto”) is an industrial diagnostic clinic that caters local businesses in different industries for their Pre-employment Medical Examination, Drug Test and Annual Physical Examinations. It runs on a web-based computer system which makes the medical results accessible to employer’s real time.

 

Nicatto is committed to safeguarding your rights as data subjects. In processing your personal data, we are guided by the principles of transparency, legitimate purpose and proportionality. Our priority is your privacy.

 

Nicatto is your assigned Pre-Employment Medical Examination (PEME) “Clinic”. Through this PATIENT REGISTRATION SYSTEM and in the course of the medical examination to be conducted on the day of your PEME, Nicatto will collect the following personal and sensitive personal information which includes your medical data (collectively “personal data”) for the purpose of your pre-employment medical examination:

 

• Information in relation to the COVID-19 Declaration Form:

  • Signs and symptoms

  • Travel information

• Personal data:

  • Name

  • Gender

  • Birthdate

  • E-mail address

  • Contact number

  • Company

• Medical History

• Medical Examination Data:

  • Chest X-Ray

  • Blood sample

  • Urine sample

  • Fecalysis Sample

  • Physical exam

 

Your personal data including the results of the medical examination will be submitted directly to your employer/prospective employer for their assessment and consideration as part of their recruitment and hiring process. Nicatto only processes your personal data and conducts the medical examination upon the instruction of your employer/prospective employer,  should you wish to have a copy of your medical examination result, you should request the same directly to your employer/prospective employer.

 

Nicatto will never share your personal data with any other third-party except:

• To third-party service providers engaged by us who process personal data on our behalf to provide us services. These service providers will only process your personal data upon our instructions;

• When you requested or when you give consent to the transfer or sharing of your personal data; or

• When we are required by applicable laws or regulations.

 

In cases when information is disclosed to another entity, Nicatto will make sure that this is on a need-to-know basis only. In such an instance, the transfer of your personal data will be done in accordance with applicable Data Privacy Laws.
